is sarafina the sister of sarabi 或者 are they 老友记 this has been bugging me since the first movie

I believe they're just friends. It would be very pretty awkward if Simba and Nala are cousins in that case. Sarabi and Sarafina are no where related, they're just 老友记 and nothing else.
