One. prestigious families, and two: wealth.

And prosperous ponies have much time on their hands. Therefore, this Ouran Host Club is about... these handsome guys that have time... giving hospitality to these lovely ladies who also have time... and profit off of them. It's an elegant game that is unique to this super-rich school.

Does YOUR school have something like Ouran? :P

There are actually private schools like that, everywhere. There's some around where I live and there's some total riches around with these humongous mansion houses. I doubt any prestigious private school around is even close to as... /interesting/ as Ouran though XD
