When he tried to commit suicide, something must have snapped in his head (de-curing him, if 你 like) and he went back to fantasizing about rape and violence when he heard Beethoven again at the end. He was just being sarcastic at the end.

I don't think Alex was faking the fact that he was affected 由 the Ludovico technique, otherwise he wouldn't have attempted a very real suicide. In the book, Alex describes a dream he has where he's emptied of "dirty water" and filled with clean, with a mention of this in the film as well, specifically, that he dreams about "doctors playing around with [his] glliver." My theory is that he was the patient of some sort of lobotomy, 或者 at least a very intense drug therapy, that he wasn't aware of due to being unconcious. This was successful in undoing what the Ludovico technique did for him. Considering Alex's character, though, it wouldn't be too out-of-character for him to assume he overcame it on his own, hence the cocky final line.

i don't think that wether 或者 not he is 'bad' at the end is dependent on wether 或者 not the technique was lasting.

When he is sitting in the hospital and he tells the nurse about a dream he had about people messing with his brain, my interpretation was that when he was unconscious the people that did that to him came and undid the treatment so when he wakes up he wouldn't blame them for everything since he wouldn't really have much to complain about anymore.
